Arranging Services

It is easy to arrange for VNA Nazareth Home Care. Our Customer Service/Referral Department is made up of nurses who can answer your questions about home health services and take referrals for services (or can direct you to the appropriate person).

Referrals for home care services are taken 24 hours a day, seven days a week. Office hours for the Customer Service/Referral Department are Monday through Friday 8:00 a.m. - 5:00 p.m.

To make a referral the following information will be needed:

  • Patient's name, address, phone number, date of birth and social security number.
  • Doctor's name, phone number and address.
  • Diagnosis of treatment required, start of care date and medications or supplies needed.
  • Insurance carrier, policy number or person responsible for payment.

To Make a Referral from the Hospital

Physicians who are making referrals from the hospital can simply write "Refer to VNA Nazareth Home Care" or VNA Nazareth Home Care Home Infusion" in the orders. VNA Nazareth Home Care has nurse coordinators at each hospital to expedite referrals and arrange for a smooth transition from hospital to home.
