Admission to the Program

Frazier Rehab offers a full continuum of care for the brain injured patient from inpatient care to outpatient treatment in the NeuroRehab Program and beyond.  The NeuroRehab Program is an intensive outpatient therapy program for patients from adolescence through the geriatric years.  Our highly qualified team treats a wide variety of patients with acquired brain injuries and neurological conditions including:

  • Aneurysm
  • Anoxia/Hypoxia
  • Brain Tumor
  • Cognitive Impairments as a result of seizure disorders
  • Encephalopathy
  • Infections
  • Metabolic Disorders
  • Mild Traumatic Brain Injury (Post-Concussion Syndrome)
  • Stroke
  • Traumatic Brain Injury

Whether you are transitioning from inpatient care, are being referred by a physician for outpatient care or are just an individual searching for answers, we will work with you to ensure you receive the best and most appropriate care possible.

Before beginning any treatment, the Frazier team talks with the patient, his/her family and his/her physician about the patient’s needs and then schedules a comprehensive evaluation, unless the patient is directly referred from an inpatient stay at Frazier Rehab.  Once the evaluation is complete, our team meets to determine the most effective course of treatment for that individual patient.

Upon admission to the program, each patient is assigned a primary therapist as his/her main contact person in the program and is assigned to a two-day, three-day or five-day-a-week track depending on the patient’s needs.  We will also work with your insurance company to pre-determine your insurance coverage before beginning the program and help guide you through the process to maximize your benefits and minimize any financial burdens.

If, however, after completing the initial evaluation it is felt that the NeuroRehab Program is not the best course of treatment for you at this time, our team will provide you with additional recommendations or referrals for more appropriate services.  These recommendations may include but are not limited to: neuropsychological evaluation, vision specialist, specialized residential brain injury treatment program, rehab physician/physiatrist, home health therapies, skilled nursing facility placement or an adult day treatment program.
